SCHEDULE
New Patients
New patients wishing to establish care with us should first complete our new patient packet. You may return this paperwork to us via email, mail, fax, or dropped off at the office. Once we receive the completed forms, our office staff will reach out to you to schedule your initial evaluation appointment.
Existing Patients
Patients who have previously established care with us may schedule follow up appointments online via our patient portal. Please note that this option is not available for new patients since the patient portal does not offer time slots for new patient appointments which take more time. We will not be able to honor initial evaluation appointments which are self-scheduled in a follow up appointment time slot.

8:00 A.M - 4:30 P.M
Friday
8:00 A.M - 2:30 P.M
Please note that we do not offer services on weekends or after-hours. If you are in crisis or experiencing a medical or psychiatric emergency, then you should call 911 or seek immediate treatment at an emergency department or urgent care facility.
